Program Overview
In order to improve the health of patients and the population and align with the trajectory towards value-based initiatives and payment structures, physicians and other providers need to expand and reorient their mental paradigms to also incorporate upstream – social and environmental – factors into their understanding, practice, and treatment. This series will address key upstream health issues currently affecting the greater Houston population.
The first topic in this series is Mental Health.

Target Audience
Physicians, nurses, case managers, social workers, allied health care professionals and others involved or with an interest in social determinants of health.

Educational Objectives

  • Upon completion of this course, the participants should be able to:
  • Describe the upstream issues affecting the health of the greater Houston population
  • Identify patients in need of community resources and the screening tools to use
  • Implement appropriate interventions and responses to the patients’ needs
